Form Spa has wonderful staff and services. They take the time to explain each step that they are taking and make sure that you are comfortable. It is a beautiful space and the professional services they offer are outstanding. I can’t wait to return and be beautified by Heather and Daphne again!

Dr. Bethany Powers corrected my deviated septum and I had such a good experience with her, I felt I had to come and share a review on what an exceptional surgeon she is. It’s scary to trust a doctor with a surgery on your face, so I vetted several surgeons before I found Dr Powers. I knew after my first consultation that I trusted her. From the initial consultation to post surgery follow ups, Dr. Powers professionalism and compassionate approach made me feel comfortable, safe, informed, and most importantly, I felt heard. I truly felt she listened to and understood my unique concerns and goals for the surgery, and took the time to explain, not only the technical details of the surgery, but also the impacts emotionally and mentally. It felt like my well-being as a whole was being taken into consideration. She is clearly a skilled expert in her field, my surgery outcome is even better than I had hoped—I can breathe again!! FANTASTIC EXPERIENCE By far the best experience I have ever had at a spa! They facility was in immaculate shape. Absolutely beautiful and well-decorated. The ENTIRE staff was amazing, helpful and kind (not just the girl performing my services). You can really feel the unity of the team, striving to create the best experience possible! Now both my husband and I go there!! We are hooked for life!! Treatments done by Kayla have always exceeded expectations, and the best person I have ever gone to! I had the most amazing experience here. I went in for a breast augmentation and from the very beginning for the consultation, to the actual procedure, I was very impressed. Dr. Chidester is so professional he made me feel so comfortable and he does amazing work. He is so personable. I highly recommend him to anyone and everyone. When I got there for my procedure, my nurse Taylor walked me through the whole process. She is so friendly and talked with me the whole time and kept me comfortable. Any questions I had, she answered. Sean the nurse anesthetist walked me through all the medications, exactly what I would experience. He was extremely friendly and helpful. The medical assistant Rachel made sure she was there when I woke up and kept me comfortable and dealt with all my weird questions. This team is nothing short of amazing. Their bedside manner is above and beyond anything I’ve ever experienced. Thank you guys. Each and every one of you really exceeded my expectations.

Dr. Bethany Powers…there aren’t enough characters in this review box to describe her work and professionalism. She performed a rhinoplasty on me in August and I am beyond satisfied with my results. Dr. Powers is intelligent, charismatic and caring. She listened to what I wanted as well as answered all my questions. She is meticulous in her work, not just on the operation table, but along with the consultation and post op appointments. So don’t just think about getting your procedure done, think about getting it done with Dr. Powers.
